MoodCheck uses “Google Forms” because, well, there’s no money coming in to pay for a fancier tool.
But the Form does not store any information that would identify you. It stores your responses so that if you came back to your questionnaire after leaving it, you could continue from where you left off.
The email address you may have noticed at the top of the questionnaire (DepressionEducationForms@gmail.com) is just there to enable use of Forms. That Google account is where your responses are stored. Thus the cute little cloud icon. Only that account, not your phone or computer, is subject to Google’s Privacy Policy.
I’ll not be using that Forms email address for any other purpose. Having made this questionnaire I’ll be able to look at scores from many users but I won’t be able to see yours individually.
